HealthInformationHealth Information Management (HIM) is responsible for maintaining medical records in a standardized and professional manner that will protect patient confidentiality while allowing adequate access to healthcare providers to promote quality patient care.

HIM services include:

  • Transcription
  • Diagnosis and procedure coding
  • Release of information
  • Providing information to state agencies concerning traumas, head and spinal injuries, and deaths

To obtain a copy of your own medical records, you must sign a Release of Information form. A parent or legal guardian must sign the release for a child or mentally incompetent patient, with required documentation. There is no charge.

We require at least 24 hours, and not to exceed 30 days, to release medical information, unless it is a medical emergency. The forms can be downloaded here on this Website.
